A CONSULTATION
Please call us at 602-993-9100 to schedule a consultation.
Your first appointment with
Dr. Chen will consist of an initial consultation. At that time, he will examine you and discuss your treatment options. Arrangements
will then be made to schedule your surgery. The initial consultation will take between 30-60 minutes.
Please arrive
a few minutes before your appointment. You will be asked to fill out a medical history and other information. If your visit is medical in nature, please bring your insurance cards. If your appointment is cosmetic in nature, the charge is $100.00, which
will be credited toward your surgical procedure.
We have offices located in Glendale and Scottsdale, to serve our
patients throughout the Phoenix metropolitan area.

Palms Medical Complex. Building B is immediately on your left.
