Schedule
Please call our office to schedule a consultation. We have offices in Scottsdale and Glendale for your convenience. Your first appointment will consist of an initial consultation. At that time, one of our doctors will examine you and discuss your treatment options. Arrangements will be made to schedule your surgery. This appointment will take between 60-90 minutes, depending upon the complexity of your problem.

Prepare
If your visit is medical in nature, please bring your insurance cards and a photo ID. You will be responsible for any applicable deductibles and copayments. If your appointment is cosmetic in nature, or if we do not take your insurance, the charge is $350, and is nonrefundable. This fee will be credited towards any services with your doctor.
You will be asked to fill out a medical history and other information. Also, remember to bring a list of your medications, supplements, allergies and previous surgeries. If you have had any recent bloodwork or EKG’s, please bring them with you.

Arrive
Please show up to your appointment at least 15 minutes before for check in and registration. We recommend reserving at least 60-90 minutes for your appointment time. This includes check-in, registration, consultation, and surgery scheduling (if applicable). If you cannot reserve that amount of time, please call our office to reschedule your appointment at least 24 hours in advance. Any cancellations and reschedules within 24 hours will result in a $50 cancellation fee.
